Contributory Election Forms
Act 2084 of 2005 requires that new members of APERS on or after July 1, 2005 contribute 5% of their earnings to the retirement system. This Act also provided an opportunity for those non-contributory members of the System to elect to be contributory. The deadline for this election was December 31, 2005. Therefore, it is no longer necessary for you to have your employees that are current members of APERS complete the Election Forms.
Your employees that are on active duty have six-months from the date that they return to covered employment to choose to become contributory. If you have members who return from active duty, you should forward us a copy of their DD214 Form so that they may be given credit in the retirement system for the period of active duty. At that time, the Counseling Staff will inform them of their choice to be contributory. Remember – each member will have six-months from the date they return to covered employment to choose to be contributory.
New Hires
Employees hired by your agency who have never been a member of APERS must complete and forward to us the following forms for membership in the System:
Former Members
Employees hired by your agency who had been non-contributory members of APERS and who have returned to a position covered by APERS within six months or less of their termination date, can remain non-contributory. They also have the option to become contributory. However, if they wish to be contributory, they must do so when they complete the MDF. In addition, the Election Form must be completed and returned with the appropriate beneficiary form (see below) as well as the MDF and a copy of their social security card.
Designation of Beneficiary Forms
It is not necessary for a member to complete both the Designation of Beneficiary Form and a Change of Beneficiary Form. Choose one of the forms to be completed based on the following descriptions:
Designation of Beneficiary Form (pdf) – This form is to be completed for new members to the System or for members who have never had employee contributions in the System.
Change of Beneficiary Form (pdf)– This form is to be completed when a member or former member has employee contributions in the System and wishes to change who they have listed as beneficiary.
Multiple Employers
Before determining that your employees are not eligible to participate in APERS, please verify that they are not employed in a position covered by APERS. If your employee works for multiple APERS employers and works at least 80 hours per month for all employers, then they should be reported to APERS by all employers with the applicable earnings, contributions and service credit. If any of your employees are employed in multiple positions covered by APERS, that employee must be either contributory or non-contributory with all employers.
